NEW PASSPORT – MINOR

Passport copies of both parents
Copy of parent’s national identity card.
Child’s original birth certificate. If not in English, it must be translated at the Embassy.Note:Birth Notification certificate and Hospital Entry forms are not accepted
Copy of parents residency permits
